Do lug nuts come included?

Lug nuts are not included with this wheel. Because the FF66 is a forged dually wheel, you'll want to verify your seat type and thread pitch — Ram 3500 applications typically require a specific shank-style lug, and using the wrong seat can compromise torque retention over time.
